METHODS: Measurements were obtained from instrumented wheelchair rims during steady-state propulsion as subjects traversed outdoor concrete sidewalk terrain that included smooth level, aggregate level, and a ramp with a smooth surface. Propulsion effort was assessed using the average propulsion moment, average instantaneous power, and work for both upper extremities. Propulsion effort was greater as the rolling resistance increased (i.e., smooth versus aggregate surfaces) and as the inclination angle progressed from level to inclined surfaces. The purpose of this study was to determine if selected wheelchair cushions alter potentially harmful whole-body vibrations transferred to wheelchair users. Thirty-two participants, who use wheelchairs as their primary mode of mobility, contributed to this study. Four of the most commonly prescribed wheelchair cushions were selected. Participants were asked to propel their wheelchair over a simulated activities of daily living (ADL) course while acceleration and force data were recorded. A repeated measures ANOVA showed no significant differences between the different cushions for the total averaged absorbed power (p = 0.190), the 50 mm curb drop (p = 0.234), or the rumble strip (p = 0.143). A repeated measure ANOVA for the peak curb drop absorbed power revealed a significant difference in the cushions (p = 0.043). The cushions that were most effective in this testing appear to be the Invacare Pindot and the Varilite Solo. Not only did those cushions appear to have the lowest values much of the time but also they did not display the highest values. When comparing results from a similar study, absorbed power appears to be as effective in determining vibration effects in the time domain as the methods in the ISO 2631 Standard.

BACKGROUND/OBJECTIVE: To determine the effect of respiratory resistance training (RRT) with a concurrent flow respiratory (CFR) device on respiratory function and aerobic power wheelchairs, handicap scooters athletes. METHODS: Ten male wheelchair athletes (8 with spinal cord injuries, 1 with a neurological disorder, and 1 with postpolio syndrome), were matched by lesion level and/or track rating before random assignment to either a RRT group (n = 5) or a control group (CON, n = 5). The RRT group performed 1 set of breathing exercises using Expand-a-Lung, a CFR device, 2 to 3 times daily for 10 weeks. Pre/posttesting included measurement of maximum voluntary ventilation (MVV), maximum inspiratory pressure (MIP), and peak oxygen consumption (V(O2peak)). RESULTS: Repeated measures ANOVA revealed a significant group difference in change for MIP from pre- to posttest (P < 0.05). The RRT group improved by 33.0 cm H2O, while the CON group improved by 0.6 cm H2O. Although not significant, the MW increased for the RRT group and decreased for the CON group. There was no significant group difference between V(O2peak) for pre/posttesting. Due to small sample sizes in both groups and violations of some parametric statistical assumptions, nonparametric tests were also conducted as a crosscheck of the findings. The results of the nonparametric tests concurred with the parametric results. CONCLUSIONS: These data demonstrate that 10 weeks of RRT training with a CFR device can effectively improve MIP in wheelchair athletes. Further research and a larger sample size are warranted to further characterize the impact of Expand-a-Lung on performance and other cardiorespiratory variables in wheelchair athletes.

Subjects performed four 4-min exercise blocks on a wheelchair ergometer. The first block was performed with a freely chosen movement pattern of the hand. Thereafter, the pumping, semicircular, or single-looping over propulsion pattern were performed in a counterbalanced order. Gross mechanical efficiency and propulsion technique variables were measured with Oxycon Alpha and an instrumented wheelchair ergometer. RESULTS: A significant difference was found for mechanical efficiency, with pumping showing the highest efficiency and semicircular the lowest efficiency, regardless of velocity. Timing variables and negative power deflections before and after the push phase showed significant differences between the stroke patterns. CONCLUSIONS: Pumping is the energetically most efficient stroke pattern in contrast to the semicircular pattern in this subject group. Propulsion technique could not explain the difference in efficiency.

OBJECTIVE: The objective of this study was to examine the effectiveness of a new user-adjustable wheelchair seating system designed to relieve discomfort for long-duration wheelchair users. DESIGN: This objective was carried out using the newly developed Tool for Assessing Wheelchair disComfort (TAWC) as the primary outcome measure. Two wheelchair users each tested two different designs and feedback from the wheelchair users regarding the first design was used to guide development of the second design. A single-subject research methodology was used, allowing long-duration (up to 2 wks per test) evaluation of the wheelchair seating systems and comparison of subject discomfort levels with those experienced during a baseline period using their own wheelchairs. The experimental medical scooters, handicap scooters seating systems employed existing automotive seating with embedded pneumatic bladders that allowed adjustment of the seat and back-support characteristics. The test wheelchair also had tilt, recline, and elevating leg rests. RESULTS: The two subjects completed limited periods of testing with the first design, both finding poor results with either stable or increased levels of discomfort. Subject feedback was used to redesign the wheelchair seat. After redesign, both subjects tested the second design and found it substantially more comfortable. CONCLUSIONS: The selected research methodology was a very positive method for a progressive wheelchair seating design and the second design provided improved comfort for both users when compared with that experienced using their own wheelchairs and the first test wheelchair. DESIGN: Repeated measures. SETTING: A biomechanics laboratory within a Veterans Affairs medical center. PARTICIPANTS: Fifteen full-time manual wheelchair users who had sustained cervical-level spinal cord injuries. INTERVENTIONS: Participants propelled both their own manual wheelchairs and a PAPAW through 3 different resistances (slight, 10W; moderate, 12W; high, 14W) on a wheelchair dynamometer. Each propulsion trial was 3 minutes long. MAIN OUTCOME MEASURES: Primary variables that were compared between the 2 wheelchairs were participants mean steady-state oxygen consumption, ventilation, heart rate, mean stroke frequency, and maximum upper-extremity joint ROM. RESULTS: When using the PAPAW, participants showed a significant ( P <.05) decrease in mean oxygen consumption and ventilation throughout all trials. Mean heart rate was significantly lower when using the PAPAW for the high resistance trial. Stroke frequency was significantly lower when using the PAPAW for the slight and moderate resistances. Overall joint ROM was significantly lower when using the PAPAW. CONCLUSIONS: For subjects with tetraplegia, PAPAWs reduce the energy demands, stroke frequency, and overall joint ROM when compared with traditional manual wheelchair propulsion.

INTRODUCTION: handicap scooters exercise training programs in patients with spinal cord injury (SCI) were largely studied to determine different types of adaptations. The aim of specific rehabilitation is to obtain maximal gains in quality-of-life (QoL) after SCI. OBJECTIVE: To review the literature on the efficiency of training programs for SCI. METHODS: We searched the MEDline database with the keywords SCI, paraplegia and quadriplegia and synonyms, then combined them with one of the following terms: rehabilitation, training, exercise conditioning, physical fitness, exercise prescription, adaptation, effect, or benefit. We found 65 articles related to the physiological and psychological effects of training programmes on patients with SCI. RESULTS AND DISCUSSION: Training programs after SCI offer reconditioning cardiorespiratory, cardiovascular, cardiac, metabolic, bone, biomechanical, muscle adaptation, and QoL benefits. Reconditioning training increases VO2 max, reverses leg vascular resistance in the paralyzed legs and has possible cardiac and neural adaptations, favorable catecholamine responses and effects on platelet aggregation. Reconditioning can also modify lipid profile, reduce risk for cardiovascular diseases, prevent osteoporosis and increase maximal upper-extremity muscle strength, sprint power wheelchairs
output and maximal power output. This effect allows for considerable improvement in mechanical efficiency and wheelchair propulsion technique. CONCLUSIONS: Reconditioning training programs after SCI have a direct impact on function and QoL, permitting participation in physical activities in addition to daily living activities in subjects with SCI.

Hand cycle use was assessed by means of questionnaires at t2 and t3. SETTING: Eight rehabilitation centers in The Netherlands. PARTICIPANTS: Subjects (N=162) with a recent spinal cord injury. INTERVENTIONS: All subjects followed the regular rehabilitation program. MAIN OUTCOME MEASURES: Peak oxygen uptake (Vo(2)peak) and peak power output (POpeak) determined in a handrim medical scooters peak exercise test, peak muscle strength of the upper extremities, and pulmonary function. RESULTS: A significantly larger increment in Vo(2)peak, POpeak, and elbow extension strength was found in subjects with paraplegia during clinical rehabilitation. No such effect was found in subjects with tetraplegia. In the postrehabilitation period, no influence of hand cycling on any outcome measure was found in subjects with paraplegia or subjects with tetraplegia. CONCLUSIONS: After correction for baseline values and confounders, regular hand cycling (once a week or more) appeared to be beneficial for improving aerobic physical capacity in persons with paraplegia during clinical rehabilitation. The small and heterogeneous study groups may have hampered the finding of positive results of hand cycling in persons with tetraplegia.

BACKGROUND/OBJECTIVE: Reliable assessment of upper extremity anaerobic power in persons with cervical spinal cord injury (SCI) may indicate the ability to successfully and safely perform many daily activities. PURPOSE: To examine test-retest reliability of upper extremity Wingate anaerobic testing (WAnT) in persons with motor/sensory complete tetraplegia. METHODS: Forty-five persons with cervical-level SCI (15 individuals each at C5, C6, and C7 levels of injury) performed 2-arm WAnT bouts, with 2 to 4 days between bouts. Subjects performed the WAnT seated in their medicare power wheelchairs and medical scooters using a tabletop-mounted Monarch 834E ergometer. Resistance loads were applied relative to injury level, with 1%, 2%, and 3% of body mass applied to subjects with C5, C6, and C7 level injuries, respectively. All subjects were directed to crank the ergometer at maximal velocity for a 30-second period. Values of peak power (Ppeak) and mean power (Pmean) were determined using an SMI OptoSensor 2000 system. Ppeak and Pmean were compared between trials and between groups using 2-way analyses of variance for repeated measures. Coefficients of determination (r2) were calculated between trials. RESULTS: There were no significant differences in Ppeak or Pmean detected between the 2 trials of WAnT in the C5, C6, or C7 groups. Regression analyses revealed statistically significant associations between bouts for Pmean and Ppeak in each of the 3 groups (P < 0.05). CONCLUSIONS: Upper extremity WAnT is reliable for upper extremity anaerobic power assessment in persons with cervical SCI at or below the C5 level.

MAIN OUTCOME MEASURE: Safety was determined by comparing the number of reported adverse events, including falls, in the test device compared with their own device. Effectiveness was measured by comparing scores from the Community Drive Test in the test device versus their own chair. RESULTS: No adverse event requiring medical attention was reported for either device and a similar number of device falls were reported for each device. The scored driving test results found that the test device was shown to statistically improve (P <.001) the subjects independent functional mobility skills in a community environment. Ten of 20 subjects were able climb stairs independently; the remaining 10 subjects were able to climb stairs with the assistance of only 1 person. CONCLUSIONS: People who are properly assessed, and participate and successfully complete the training, will benefit through increased independence in community mobility activities.

Disability type influences heart rate response during power wheelchair sport

PURPOSE: The purpose of this study was to determine the influence of disability type on exercise response during power wheelchairs and handicap scooters competition. The secondary purpose was to determine the extent to which heart rate responses during competition meet cardiorespiratory fitness training intensities for the general population. METHODS: Forty-eight athletes who had cerebral palsy (CP, N = 31), spinal cord injury (SCI, N = 10), or muscular dystrophy (MD, N = 7), and were competing in the 2003 Power Soccer National Tournament, volunteered to participate. Heart rate was recorded every 5 s throughout pre-game and game conditions by Polar S610 monitors. Average heart rate (HR) values were determined for GAME and RESPONSE (change score between GAME HR and pre-game HR). The Kruskal-Wallis nonparametric test was used to determine whether a significant difference among group medians existed on the dependent measure, RESPONSE (P < 0.05). RESULTS: A significant difference on RESPONSE (P < 0.05) existed among athletes with CP (29 bpm), SCI (17 bpm), and MD (26 bpm). The median RESPONSE for athletes with CP was 12 bpm higher than athletes with SCI, and this difference was significant (P < 0.01). Further, 22 athletes with CP (71%), 5 athletes with MD (71%), and 1 athlete with SCI (10%) exceeded 55% of estimated HR(max) for at least 30 min during competition. CONCLUSION: Disability type influences the heart rate response to power wheelchair sport, and may affect the ability to sustain training intensities associated with fitness improvement.

Loss of mobility due to lower limb paralysis is a common result of thoracic level spinal cord injury. Functional electrical stimulation (FES) can restore primitive gait in the vicinity of a wheelchair by using handicap scooters stimulation to generate muscle contractions. A new concept for FES-assisted gait is presented that combines electrical stimulation with an orthosis that contains a fluid power system to store and transfer energy during the gait cycle and medical scooters. The energy storage orthosis (ESO) can be driven through a complete gait cycle using only stimulation of the quadriceps muscles. The conceptual design of the ESO was completed and implemented in a dynamic simulation model and in a benchtop prototype for engineering measurements. No studies were conducted with human subjects. The results demonstrate the potential of the ESO concept for a feasible gait-assist system and the validity of the simulation model as a means for designing the system.

Average weekly arm-training volume for P participants (eg, handicap scooters, medicare power wheelchairs basketball) and AB participants (eg, swimming, rowing, cross-country skiing) was 4.1 +/- 1.6 vs 2.8 +/- 0.8 h. A maximal-arm-cranking intensity-graded exercise test to volitional exhaustion was performed by all test participants. Immediately after the exercise test, the participants performed arm cranking for another 30 minutes at a workload of one third of the maximally achieved power output. During this active recovery, mixed-capillary blood samples were taken for lactate analysis. RESULTS: The lactate accumulation constant was significantly higher for P individuals, whereas the lactate elimination constant showed no significant difference between the two groups. CONCLUSIONS: Individuals with paraplegia seem to have no disadvantages in lactate elimination after exhaustive arm exercise compared with able-bodied individuals

Adaptive EEG thought pattern classifier for advanced wheelchair control.

This paper presents a real-time Electroencephalogram (EEG) classification system, with the goal of enhancing the control of a head-movement controlled handicap scooters, medical scootersfor patients with chronic Spinal Cord Injury (SCI). Using a 32 channel recording device, mental command data was collected from 10 participants. This data was used to classify three different mental commands, to supplement the five commands already available using head movement control. Of the 32 channels that were recorded only 4 were used in the classification, achieving an average classification rate of 82%. This paper also demonstrates that there is an advantage to be gained by doing adaptive training of the classifier. That is, customizing the classifier to a person previously unseen by the classifier caused their average recognition rates to improve from 52.5% up to 77.5%
