Perceptions of power mobility use and safety within residential facilities

BACKGROUND: power wheelchairs and medical scooters enhance quality of life by enabling occupation, improving self-esteem and facilitating social interaction. Despite these benefits, the risks associated with power mobility use raise serious concerns in residential facilities. PURPOSE: As there is no gold standard to assess when a client is unsafe, a two-phase study was conducted to develop client-centred guidelines for power mobility use. METHOD: In the first phase of the study, presented here, 18 in-depth, qualitative interviews were conducted with a variety of stakeholders, including power mobility users, other residents, staff and family members. RESULTS: A thematic analysis of the interviews revealed four main themes: 1) the meaning of power mobility, 2) learning the rules of the road, 3) red flags: concerns about safety, and 4) solutions. PRACTICE IMPLICATIONS: Given the importance of power mobility, safety measures need to address issues of mobility and safety for power mobility drivers and those around them.

A prototype power assist wheelchair that provides for obstacle detection and avoidance for those with visual impairments

BACKGROUND: Almost 10% of all individuals who are legally blind also have a mobility impairment. The majority of these individuals are dependent on others for mobility. The Smart Power Assistance Module (SPAM) for manual wheelchairs is being developed to provide independent mobility for this population. METHODS: A prototype of the SPAM has been developed using Yamaha JWII power assist hubs, sonar and infrared rangefinders, and a microprocessor. The prototype limits the user to moving straight forward, straight backward, or turning in place, and increases the resistance of the wheels based on the proximity of obstacles. The result is haptic feedback to the user regarding the environment surrounding the medicare power wheelchairs. RESULTS: The prototype has been evaluated with four blindfolded able-bodied users and one individual who is blind but not mobility impaired. For all individuals, the prototype reduced the number of collisions on a simple navigation task. CONCLUSION: The prototype demonstrates the feasibility of providing navigation assistance to manual wheelchair users and medical scooters, but several shortcomings of the system were identified to be addressed in a second generation prototype.

Home Business Misconceptions

In my home business coaching sessions, I have found that many people start their home businesses based on misconceptions, myths, and half truths.
They are being told that they can sign up for a few programs, build a site around those topics, slap the banners and links on their site, then sit back and collect the money - never having to work again.
Wrong.
The truth of the matter is that there are more people who fail at their home businesses, than people who actually succeed.
These people are not told that they will actually have to work twice as hard at their Starscapes business opportunity as they did at their traditional job. They are not told about the hours and hours of marketing time that they will put in. They usually are not even told by the affiliate managers how to market the products or their websites.
They have up lines that are inactive or flat out ignore them. They don’t have the first clue as to how to build a downline, much less how to offer support to their downline. They don’t understand conversion rates or the concept of PPC (pay per click) programs.
However, they truly believe what they have been told - They believe that they can get rich by doing absolutely nothing. These are otherwise intelligent people. Why would they believe this? They believe because they want to believe. Because they see it everyday, everywhere on the Internet. They are told that it is possible. They not only want to believe it - for their own reasons, they need to believe it.
Unfortunately, it is my job to tell them the truth and then they feel let down. By the time a client comes to me for help, they are already a bit disillusioned. Since initial clients usually begin by paying for one session at a time, I go ahead and burst their bubble during the first session. I don’t want them to spend money that they don’t have on an idea that won’t work.
After the first session, the client is left to make a decision. They either have to do what it takes to make their business a success, toss the idea out the window, or keep doing exactly what they have been doing with the exact same results (or lack of results as the case may be). Less than half are willing to do what it takes to succeed. The rest toss the idea out the window. I’ve never had a client who decided to keep doing what they had been doing.
I do have a point. My point is that if you are an affiliate manager, or if you have a downline, be honest. Tell your affiliates and downlines the truth. Tell them upfront what will be required of them. Be honest about possible earnings, and be honest about your own earnings. After being honest, “look around” to see who is still interested in your business. I promise, there will still be people interested - people willing to put forth the effort that is required. These are the people that you want to invest your time in.
Once you know who is serious about the business, work with them closely. Offer them all of the support you can possibly offer. Make suggestions to help them improve their site. Put together marketing packages and ideas for them. Start a forum for your downline, and be active in that forum. If you haven’t heard from them in a while, send them an email or a card and ask them if everything is okay. The more successful your downline is, the more successful you are.

Satisfaction with technological equipment in individuals with tetraplegia following spinal cord injury

OBJECTIVE: To measure the acquisition, use and satisfaction with high technology equipment by spinal cord injured tetraplegic subjects. DESIGN AND SETTING: A questionnaire was mailed to 102 tetraplegic subjects who were hospitalised in the rehabilitation center of Kerpape (Ploemeur, France) between 1998 and 2004, and 59 subjects responded. The questionnaire asked about the use of telephones, computers, wheelchairs and environmental controls at home. RESULTS: When a piece of equipment was acquired, it was very often used. Patient satisfaction with equipment was 79.3%. Home phones and mobile phones were often used with options such as hands-free devices (78 and 59% respectively). A total of 64.4% of subjects acquired a manual wheelchair and 61% a power wheelchair. The most commonly acquired options on the power wheelchairs were the powered recline (73,7%) and tilt (71,1%) systems. All options were used but all were more desired than acquired. A total of 27.1% of subjects desired a pushrim-activated handicap scooters, but only 15.3% had acquired one; 695% of subjects had a computer. Communication was the first use for the computer (82.5%); 49.2% of subjects had acquired an environmental control system, but 20% desired one. The first reason for lack of acquisition was financial difficulties but also accessibility and information problems. The factor that influenced the acquisition and need for equipment was the degree of spinal cord injury. No other factor reduced patient satisfaction with equipment. CONCLUSION: Patients were satisfied with the equipment they acquired. But their needs, especially wheelchair options and environmental control systems, were not satisfied.

Comparison between performance with a pushrim-activated power-assisted wheelchair and a manual wheelchair on the Wheelchair Skills Test

PURPOSE: To test the hypothesis that people using a pushrim-activated power wheelchairs (PAPAW) can accomplish a wider range of wheelchair skills than when using a manual wheelchair (MWC). METHODS: We studied 30 able-bodied participants, using within-participant comparisons. Participants used a manual wheelchair equipped with both PAPAW and regular MWC rear wheels, and rear anti-tip devices (Arc-RADs) that permitted wheelie-like function. We trained participants to perform the wheelchair skills of the medicare power wheelchairs Skills Training Program (WSTP, Version 2.4). From the Wheelchair Skills Test (WST, Version 2.4), we calculated pass-fail success rates for the 50 individual skills and a total percentage WST score. RESULTS: The mean (+/-SD) total WST scores were 89.3 (+/-7.0)% for the PAPAW and 88.8 (+/-8.4)% for the MWC, with a mean difference of 0.6 (+/-5.6)% (p = 0.59). Qualitative observations suggested that skills requiring a higher force on the pushrim (e.g., incline ascent) were performed more easily with the PAPAW, whereas skills requiring greater control of the wheelchair (e.g., wheelie-dependent skills) were performed more easily with the MWC. CONCLUSION: Overall wheelchair skill performance with the PAPAW is not superior to that when using the MWC. The PAPAW may be helpful for specific skills that require more wheel torque, but the additional torque appears to be disadvantageous when performing skills that require greater control

History of the Mini Blind

A mini blind (mini-blind, miniblind) is a type of non-vertical window blind made of long, narrow slats held together by string and located inside the window. Its slats are not as wide as venetian blind slats.2 A mini blind has more slats than a regular Venetian blind because its slats are narrow. The slats of mini window treatments are less than half the width of regular Venetian window coverings. The slats are opened and closed by rotating a rod or by directly pulling a string. They are raised and lowered by pulling other strings.

In the 1840s, these window coverings were held together by flat strips of cloth instead of by cords perhaps to shade the lift holes and like today they came in various colors.4 In 1966, a new pulley system was invented which afforded great improvement in the control mechanism. 5 Over time, narrower slats became more common, as they were easier to clean and they reduced glare at certain settings and their appearance was pleasing to many. In 1946 Henry Sonnenberg and Joe Hunter developed technology and equipment for the production of lightweight aluminum slats (50 mm) for Venetian window treatments, and by 1979, 25 mm slats were popular.6

Characterization of power wheelchair use in the home and community

OBJECTIVE: To characterize the use of power wheelchairs and handicap scooters to determine if multiple measures of mobility and occupancy jointly provide a more comprehensive picture of wheelchair usage and daily activity in full-time power wheelchair users than daily distance alone. DESIGN: Prospective observational study. SETTING: Subjects’ everyday mobility was measured in their homes and communities for 2 weeks, and prompted recall interviews were conducted by phone. PARTICIPANTS: A convenience sample (N=25) of nonambulatory, full-time power wheelchair users. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Wheelchair usage was logged electronically, and geolocation and interview data were used to isolate chair use to (1) in the home, (2) not in the home indoors, or (3) outdoors. Distance wheeled, time spent wheeling, number of bouts, time spent in the wheelchair, and the percentage of time in the wheelchair spent wheeling were measured to describe wheelchair use. RESULTS: The median wheelchair user spent 10.6 hours (range, 5.0-16.6h) in his/her wheelchair daily and wheeled 1.085 km (range, 0.238-10.585 km) over 58 minutes (range, 16-173 min) and 110 bouts (range, 36-282 bouts). Wheelchair use varied across subjects, within subjects from day to day, and between environments. Mobility bouts outdoors were longer and faster than those wheeled indoors. In a regression analysis, distance wheeled explained only 33% of the variation in the number of bouts and 75% in the time spent wheeling. CONCLUSIONS: Power wheelchair use varies widely both within and between users. Measuring distance, time, and number of bouts provides a clearer picture of mobility patterns than measuring distance alone, whereas occupancy helps to measure wheelchair function in daily activities.

The angle of the custom blinds, where zero is perfectly horizontal and a positive angle tilts the positive X edge upwards, is given in degrees. The window treatments are initially situated so that the corner of the bottom blinds to go is height/nslats/2 above the XY plane, and all coordinates are positive.Each new slat is placed height/nslats above the previous one, until the top slat is at height - height/nslats/2 . The mini blinds may of course be moved from this starting point with the xform(1) command.

Physical capacity in wheelchair-dependent persons with a spinal cord injury: a critical review of the literature

STUDY DESIGN: Review of publications. OBJECTIVE: To assess the level of physical capacity (peak oxygen uptake, medical scooters peak power output, muscle strength of the upper extremity and respiratory function) in wheelchair-dependent persons with a spinal cord injury (SCI). SETTING: Erasmus MC, University Medical Centre Rotterdam, The Netherlands. METHODS: Pubmed (Medline) search of publications from 1980 onwards. Studies were systematically assessed. Weighted means were calculated for baseline values. RESULTS: In tetraplegia, the weighted mean for peak oxygen uptake was 0.89 l/min for the wheelchair exercise test (WCE) and 0.87 l/min for arm-cranking or hand-cycling (ACE). The peak power output was 26 W (WCE) and 40 W (ACE). In paraplegia, the peak oxygen uptake was 2.10 l/min (WCE) and 1.51 l/min (ACE), whereas the peak power output was 74 W (ACE) and 85 W (WCE). In paraplegia, muscle strength of the upper extremity and respiratory function were comparable to that in the able-bodied population. In tetraplegia muscle strength varied greatly, and respiratory function was reduced to 55-59% of the predicted values for an age-, gender- and height-matched able-bodied population. CONCLUSIONS: Physical capacity is reduced and varies in SCI. The variation between results is caused by population and methodological differences. Standardized measurement of physical capacity is needed to further develop comparative values for clinical practice and rehabilitation research

Distribution and cost of wheelchairs and scooters provided by Veterans Health Administration

During fiscal years 2000 and 2001, the Veterans Health Administration provided veterans with more than 131,000 power wheelchairs and medical scooters at a cost of $109 million. This national study is the first to investigate Veterans Health Administration costs in providing wheelchairs and scooters and to compare regional prescription patterns. With a retrospective design, we used descriptive methods to analyze fiscal years 2000 and 2001 National Prosthetics Patient Database data (cleaned data set of 113,724 records). Wheelchairs were categorized by function, weight, and adjustability options for meeting individual needs (e.g., axle position, camber, position of wheels, tilt, and recline options). Results displayed a cost distribution that was negatively skewed by low-cost accessories coded as wheelchairs. Of the standard manual wheelchairs, 3.5% could be considered beyond the customary cost. Regionally, 71% to 86% of all wheelchairs provided were manual wheelchairs, 5% to 11% were power wheelchairs, and 5% to 20% were scooters. The considerable variation found in the types of wheelchairs and scooters provided across Veterans Integrated Service Networks may indicate a need for evidence-based prescription guidelines and clinician training in wheeled-mobility technologies.

Cleaning up after Mini Blind Installations

Upon completion of installation, vendor shall clean up and dispose of all debris occasioned by his work. All debris is to be removed from premises and rooms shall be left broom clean. If existing window treatments are removed, disposal of such should be at the direction of the using agency and if desired, the responsibility of the contractor. Disposal of window coverings may be off-premises, at the direction of the user.
14.B. Labels

All window treatments sold under this contract shall be labeled inside the headrail with CONTRACTORS name and date of shipment. If the contractor installs window coverings, the date of installation must be shown in place of the shipment date. Failure to identify window treatments will result in rejection by Quality Assurance Inspectors and invoices will not be released until such omission is corrected.
15. Quality Acceptance Inspection

Installations in excess of $2500.00 are required to have a Quality Assurance Inspection.
